The popular website FreePrintableMedicalForms.com has grown to include more than 700 medical forms and other health-related printables.

"Like everything at this site I started in 2009, there's a free version of each medical form, along with a premium, editable option for a few dollars," said Kevin Savetz, the site's creator. "There are printables for home health needs, medical offices, schools and more."

Two dozen new forms, charts, journals, trackers and other printables were recently added to FreePrintableMedicalForms.com.

The new trackers and logs include: vaccination logs, insurance claims and copay trackers and an INR anticoagulation record for people on blood thinners. A certificate of head lice treatment has been added as well, and will be useful for parents as well as schools. There are also two new variations of menstrual cycle trackers.

Also new are several journals and forms for caregivers, including an emergency treatment tracker, a hospital admission tracker, a caregiver contacts list and a medical contacts list.

The newest letter templates at FreePrintableMedicalForms.com address: a prescription request, a college dorm accommodation request, a college insurance plan waiver, a medical bill late payment request and a complaint template for cases when a pharmacist wrongly refuses to fill a prescription.

Veterinary forms round out the new additions. There's a pet vomit tracker, an emergency vet contact list, and bold, red window signs that let rescuers know there are pets inside in case of fire or another emergency. Also new are red tags that can be laminated and attached to a dog's harness to let passersby know when a rescue pet needs space.

FreePrintableMedicalForms.com also has wallet cards, exercise charts, nutrition forms, logs for people with diabetes and medication trackers as well as forms that are relevant during the coronavirus pandemic.

It's free to download and print anything at the site as a PDF to write on by hand. For those who need a more flexible form that can be customized in Microsoft Word or another compatible program, a DOC version is just $3.99 each. Especially convenient for medical offices is the all-in-one-download of the entire medical forms collection for $99.

For even more forms, there's BusinessFormTemplate.com. The site has more than 1,600 business forms, bills of sale, letters, invoice templates, real estate forms and expense reports.
