Most organizations have safety policies. They train their employees in various safety issues. They track incidents. So why do incidents still occur?

Despite all these organizations are doing proactively to promote safety, there is still a gap between what the “rules” are and what’s actually happening in the workplace. In the real world, safety is about people’s behavior.

And while most people’s risky behaviors aren’t intentional, they can get inadvertently reinforced by various pressures, expectations, or signals from leadership or from within the work environment.
