Golden CO- Caregiving expert Pamela D. Wilson hosts The Caring Generation® family talk radio program for caregivers and aging adults this coming Wednesday, August 12, on the Bold Brave Media Network. The program airs live at 9 p.m. EST. The Caring Generation® aired initially from 2009 to 2011 on 630 KHOW-AM in Denver, Colorado.

Care Transitions: Nursing Homes & Rehab What Caregivers Don't Know

Elderly parents who go to the hospital, experience what the healthcare system calls care transitions to return to home or go to a nursing home or rehab facility.  What caregivers don't know is that when care transitions are not managed appropriately—with attention to detail—elderly parents can experience greater harm to their health and well-being.

Some consumers are surprised to learn that nursing homes, skilled nursing facilities, SNFs, and rehab facilities are words for the same type of care community.  The difference is that some communities focus on rehabilitation to return patients to their homes while others offer a mix of rehabilitation and long-term care.

Long term care is when patients become permanent residents because they are unable to return home due to health concerns. In these situations, the elderly spend retirement savings to pay for monthly rates in a nursing home that can range from $7-10,000 per month until they are able to apply for government assistance called Medicaid. 

Family caregivers can struggle to help elderly parents stay at home Most elderly prefer the comfort of home versus living in a care community. However, when care needs increase a permanent move to a nursing home may be the only alternative. 

Guest Interview: Rehabbed to Death with Dr. Lynn Flint

The special guest for this program is Dr. Lynn Flint Dr. Flint is an associate professor in the division of geriatrics at UCSF. She is the program director for the UCSF integrated geriatrics and palliative care fellowship. She is a palliative care consultant and nursing home primary care physician at the San Francisco VA Hospital. She is passionate about teaching medical trainees of all levels about the intersection between serious illness communication, nursing home care, and health policy.
